Treat each environment as a gate: to promote from development to test, require clean static analysis for critical findings and a generated software bill of materials attached to the artifact. To promote to staging, add clean composition analysis for dependencies and proof of dynamic checks on exposed endpoints. To promote to production, require signed commits, signed artifacts, verified provenance, change approvals, and a rollback plan rehearsed at least once. Rather than big-bang approvals, require signoffs at the point of change: a reviewer acknowledges the threat modeling notes, a security owner confirms exceptions are documented with expiration, and the release approver validates that controls passed for the target environment. Each signoff lives in the pull request, the build system, or the change ticket, leaving a timestamp and an identity. Provide a “security summary” file in the repo that links to these artifacts for the last few releases—design notes, scans, test runs, exceptions, and approvals—so anyone can audit what was accepted and why. This avoids the bottleneck of waiting for a central team to re-review work already captured, while preserving accountability that is easy to verify later.
Continuous improvement comes from turning incidents and near misses into better gates, templates, and defaults. After an issue, run a short, blameless review focused on three questions: where did our lifecycle allow this to happen, what small change would have caught it earlier, and how will we prove that change now exists. The answers often point to updating a checklist, tightening a linter rule, adding a dependency source allowlist, or adjusting remediation windows.
